ALH School Homework Policy

 

Extended Leave Form

Please fill out this form if you will be taking your child out of school for an extended period of time. This form mist be submitted to the Principal two weeks prior to taking the leave of absence.

ALH School Medicine Administration Form

This form must be filled out each time your child needs to take/receive medication at school. We will not administer any medication without this form.
